Notes
The Ontario Hockey Association was formed in 1890 to govern amateur ice hockey in Ontario. At first, the OHA had one league of senior men's hockey teams. This group included teams from Ottawa, Kingston, Toronto, and London. For the first three years, this group would play a series of elmination playoffs leading to a single-game final playoff. For the first three years the Ottawa Hockey Club was the champions, winners of the Cosby Cup. In 1894, the Ottawa team and the Association came to a disagreement over the venue of the finals, and Ottawa left the league. It is uncertain when Waterloo joined the league.

Kitchener-Waterloo Record article of October 31, 1975 states that Edward F. Seagram was Captain of the Waterloo intermediate OHA champions in 1899, but the club was disqualified for using a player considered a "pro" and the championship went to Listowel.

Charles Albert Liffiton (1878-1941), one of the "Little Men of Iron" who won the Stanley Cup in 1902 with the Montreal Amateur Athletic Association, played with Waterloo for two seasons, 1897-1898 and 1898-1899.
