Your card cannot be used outside of the Province of Ontario. How do you use your card for drugs? Be sure to tell your doctor or dentist that you are eligible for Drug Benefits before a prescription is written. Your doctor or den- tist should have your drug card number when he or she phones your pharmacy regarding a pre- scription. When you are going to the pharmacy to have a prescription filled, be sure to show the card to your pharmacist, otherwise you may have to pay for medication. Do not leave your plastic card with your pharmacist. You may need it for other privilege services or at a- nother pharmacist. Who is Eligible for Drug Benefit? Drug benefit in Ontario applies to persons 65 and older who receive the Federal 0ld Age Security; or the Guaranteed Annual Income Sy- stem (GAINS) cheque; or who have resided in Ontario for the past 12 months. If you have resided in Ontario for the 12 months, but do not recieve 0ld Age Security or GAINS cheque, you must apply for a numbered card for Drug Benefits. What if you Lose Your Privilege Card? you lose your card, then you must write Ontario Senior Citizens Privilege Card, Benefit, Queen's Park, Toronto, Ontario 253. Privilege Card Without a Number. If don't qualify for Drug Benefit, but are 65 or over and a resident, but have not lived in Ont- ario for 12 months, you are still eligible to receive an Ontario Senior Citizens Privilege Card without a number. It won't entitle you to free durgs but will entitle you to the oth- er available privileges. To obtain an unnumbered Privilege Card, you must complete an application, a copy of proof of age is required such as your birth certifi- cate, passport or Drivers License. As in other instances, you should record your card number and keep it at home so that you have a record of your 'mEber in case your card is 'lost.
