CONTINUING CARE CO-ORDINATOR The Thunder Bay District Health Council is seeking a Continuing Care Co-Ordinator to work with the Long Term Care Committee and assist hospitals and health care agencies and programmes to co-ordinate their services on the primary care level. Responsible to: Executive Director Thunder Bay District Health Council | The Candidate shall possess: a Baccalaureate Degree in a field related to health care, at least two years' clinical expertise, an interest in gerontology and long term care, and a knowledge of community re- sources Areas of Responsibility: 1. Assessment and programming of people re- quiring care in institutions or commun i ty=- | based programmes 2. Assist with implementation of a discharge planning programme 35.
