Page 4, TO ALL LONGLAC EMPLOYEES How well do you know your Group Life Insurance Plan: Are you aware of the following facts: Every full-time employee is eligible for coverage under the Basic Insurance plan immediately he or she joins the organization, This is a coverage of $2000.00 costing the employee only 20¢ per month, : At the end of six months service all male employees (and female employees earning #2000.00 yearly or over) may take additional insurance, the amount of which is based on yearly earnings at a rate of 50¢ per $1000.00 coverage per month e.g. if earning $2000.00 but less than $3000.00 the employee qualifies for $3000.00 coverage which costs $1.50 per month. ; If an employee becomes totally and permanently disabled so that he is unable to work during the remainder of his lifetime (provided he is less than 60 years of age) further premiums are waived and his insurance paid either in one sum six months after receipt of proof of disability, or in 60 monthly installments, the first of which is payable upon receipt of proof of disability. The employee, on leaving our employ, has 31 days to convert to any type of policy then being issued by the Insurance Company (other than Term Insurance) without medical examination, If an employee has not taken advantage of coverage when first offered, he may, by taking a medical examination apply for such insurance.
